Comprehensive Physical Therapy Evaluation $225
Individual Physical Therapy Treatment Visit $135
Includes 75 minutes of testing, clinical observations, full body movement assessment, detailed review of records, and development of a personalized treatment plan. This price applies to your first visit.
Includes 45 minutes of direct treatment including manual therapy, movement training, education and guidance for your daily home program, individualized to your condition and movement goals. This price applies to follow-up visits.
Additional Services
Physical Therapy Re-Evaluation $175
A Physical Therapy Re-Evaluation may be required if (1) there are new clinical findings such as a newly diagnosed related condition, 2) for a significant change in condition or functional status which was not anticipated in the original plan of care. Typically requires 60 minutes.

Cancellation Policy
I understand that sometimes unforeseen circumstances such as emergencies, illnesses, or transportation issues can arise. However, to ensure that I can accommodate all my clients effectively, I have established the following cancellation policy:

24-Hour Notice Requirement: Cancellations must be made at least 24 hours ahead of a scheduled appointment. This allows me to offer the time slot to another client who may need my services. Cancellations made within 24 hours of a scheduled appointment will be considered late cancellations.

Late Cancellations and Missed Appointments: If you cancel your appointment within 24 hours of the scheduled time or miss your appointment, you will be subject to a cancellation fee of $75. This policy helps me manage my schedule and ensures that I can see other patients.
