Functional Embodiment Physical Therapy, PC is a cash-based practice and does not bill insurance directly. The practice is considered out-of-network with all insurance companies and is not enrolled in Medicare.
Payment Options: I accept cash, check, or credit card for payment at the time of service.
Superbills: Upon request, I can provide a detailed receipt (superbill) after your session, which you may submit to your insurance company for possible out-of-network reimbursement.
Medicare Patients: Per Medicare law, I am unable to provide cash-based physical therapy services to individuals with Medicare coverage.
This model allows me to provide personalized, one-on-one care without the restrictions or delays often associated with insurance reimbursement.
No referral is needed! California is a direct access state, which means you can schedule a physical therapy evaluation without waiting for a physician’s referral.
Direct access means:
Initial Care: You may start right away with a comprehensive evaluation and treatment.
45-Day / 12-Visit Rule: By law, you may be seen for up to 45 calendar days or 12 visits (whichever comes first) without a physician’s referral.
Continuing Beyond That: If you need physical therapy care beyond this period, you must be evaluated by a physician to ensure all necessary medical oversight is provided. To continue physical therapy beyond the initial direct access period, your physician must provide a signed plan of care, approving the physical therapist's treatment plan, which you'll bring back to continue physical therapy.
I am happy to coordinate with your physician or other providers at any point in your care if needed. While a referral is not legally required to start therapy, some insurance companies may require one for reimbursement. If you plan to submit for out-of-network benefits, it’s best to check first with your insurance provider.
This access ensures you can get started quickly, while still keeping your broader healthcare team involved when necessary.
In preparation for your visit:
Please complete the intake paperwork via the secure online portal sent to you your email address. Submit the forms 24 hours prior to your visit. This ensures all necessary information is ready for your appointment, allowing me to provide the best care possible.
How the intake paperwork (digital) process works:
Receive the Form: After scheduling an appointment, you will receive an email with a link to your secure "My Account" portal. If you are a new patient, you will be prompted to create a username and password.
Fill Out the Form: Once logged in, you will see any pending intake forms. Complete the forms securely through the online portal.
Submit the Form: After submitting the completed forms, the information is automatically saved and synced with your patient profile in our system.
Wear loose, comfortable clothing to the appointment.
Arrive 5-10 minutes early. Relax in the waiting area in Suite 2-J, and Amanda will come find you when it is time for your appointment.
Your first visit is a comprehensive 75-minute evaluation. During this session, you can expect:
A detailed conversation about your health history, goals, and current concerns
A physical assessment of posture, movement, strength, and mobility
Discussion of findings and a personalized treatment plan tailored to your needs
An introduction to hands-on therapy and movement strategies designed to give you practical tools for enhancing your movement, well-being, and overall health.
This session sets the foundation for your care, ensuring that each follow-up treatment is focused, effective, and aligned with your goals.
Functional Embodiment Physical Therapy is conveniently located in Encinitas, CA, just off Encinitas Blvd at the intersection of Village Park Way and Mountain Vista Drive. You’ll find the office inside the Park Plaza Executive Suites, Suite 2-J, on the second floor.
Parking: A parking lot is located immediately west of the building. Also ample street parking is available along Mountain Vista Drive.
Accessibility: If you require level ground access or prefer to avoid stairs, you can use the upper lot to the east of the building and enter through the back entrance. Please text Amanda to be let in, as this is a locked entrance.
Building Entrance: Access the main building through the red lobby door. Continue through the lobby and take the stairs to the second floor to Suite 2-J.

Typically, sessions last 45 minutes and are scheduled weekly.
The duration of physical therapy varies depending on an individual's condition and goals. The overall length of the therapy process can range from a few weeks to several months, depending on the complexity of the condition and the progress made. During your initial evaluation, I will assess your specific needs and provide an estimated timeline for your therapy.
Consistency in your daily home exercise practice is key to achieving lasting results and is an investment in your long-term movement well-being.
